If you are unable to complete the forms online, please plan on coming in at least 20 minutes prior to your appointment to fill out the forms at the office. Scaling & Root Planing (SRP) (up to four quads if deemed necessary)
Perio Maintenance (once every 3 months)

25% off all in-house restorative treatment necessary *
20% off in-house teeth whitening treatments.**
50% off Oral-B Tooth electronic tooth brush. Limit one per membership.**
40% off Perio Protect Treatment
*All basic and major restorative treatments performed at OCDC: includes crowns, core build up, fillings, denture fabrication and repair, night guards, root canal therapy, and extractions. Some procedures need to be handled by a specialist. Discount does not apply to any work referred out to any specialist. Additionally, discounts cannot be applied to Invisalign or Perio Protect Treatments.
